The REQUITE study

CRFs (Case Report Forms)

The primary method of data capture in REQUITE is the centralised online database. CRFs (Case Report Forms) should be completed at routine clinic appointments or in conjunction with a telephone call.

The REQUITE CRFs are copyrighted and permission must be sought to use them. Please complete your details in the CRF request form (click here) indicating which CRFs you would like access to and their intended use.  You can send the request form to  Christopher Talbot: cjt14@leicester.ac.uk (REQUITE Project Manager). You can find the watermarked Breast CRFs at the bottom of this page.

PRO (Patient Reported Outcome Questionnaires)

Patient Reported Outcome Questionnaires are not copyrighted and are free to use, nevertheless, we are interested in finding out how they will be used by other groups.

Please complete your details in the PRO request form (click here)  indicating which questionnaires you would like access to and their intended use.  A scoring manual is also available on request. You can send the request form to  Christopher Talbot: cjt14@leicester.ac.uk (REQUITE Project Manager).

We can’t provide the EORTC, MFI and GPAQ questionnaires. You can request access to use these directly from the owners, see the links below.

CRF and PRO Time points for breast patients specifically include:

• Baseline (prior to start of radiotherapy or within first five days of radiotherapy)
• End of radiotherapy (1-2 days prior to or the last day of treatment)
• 12 months from start of radiotherapy (+/- 4 weeks)
• 24 months from start of radiotherapy (+/- 4 weeks)
